Personal Services Invoice (PSI) (Word)

The Personal Services Invoice is used for Personal or Professional Services with a single payment of no more than $4999.99 with the following limitations:

• No more than five individual projects, or a cumulative total more than $4999.99, with the same contractor per department, per calendar year.
• Not for non-U.S. Citizen contractors.
• Not to be used when contractor requires their own contract to be signed.
• Not to be used for architectural or engineering services.
The PSI may be completed and sent directly to Payables for processing.

Personal/Professional Services Contract is used for Personal or Professional Services of $5,000 or greater, for non U.S. Citizen workers, for projects with multiple payments, or when the contractor requires their own contract to be signed. This form is not to be used for architectural or engineering services.

The amendment form is for use when adding money or time to a non-expired Personal/Professional Services Contract. This form should only be used in the event of an unforeseen expansion of the scope or delay in performance (not as a means for issuing a new contract with a unique scope of work).
